Advance Care Planning Group Medical Visits
UCHealth now offers Advance Care Planning Group Medical Visits for patients. The group visits are run by experts in the field of advance care planning and provide attendees with the unique opportunity to learn more about advance care planning and receive the resources to complete their documents. The group visits also act as a time to have an open discussion with peers about questions and concerns related to advance care planning.
Medical Durable Power of Attorney in the UCHealth My Health Connection
UCHealth now offers patients the opportunity to complete their Medical Durable Power of Attorney (MDPOA) through the My Health Connection portal. This important document is then uploaded directly into the patient’s medical record and is available to providers within the UCHealth system if ever the need should arise. Advance Care Planning Certification Training
Through a Denver Hospice-University of Colorado community-academic partnership, we created and offer training for community members who desire to learn more about advance care planning conversations for themselves and fellow Coloradans (funded by The Colorado Health Foundation).
Starting in 2018, we are funded by the Next Fifty Initiative to expand the program to support advance care planning conversations among older LGBT Coloradans. Click here for more information about upcoming training classes.
