We created this website with the goal of helping Coloradans find information they want about advance care planning. We want your input and love hearing about advance care planning activities and resources happening across Colorado. Please let us know if a link is not working, and we will work to update quickly.
Colorado Care Planning is supported by Dr. Hillary Lum and colleagues at the University of Colorado School of Medicine. Through collaborations and stakeholder engagement, her team works to improve and increase opportunities for advance care planning conversations in communities, clinics, and hospitals.
